Is a visa needed in addition to an approval for admission?
Yes. A visa is required to enter Germany. The German Embassy in Kabul is currently closed. The current exit route is via Pakistan as a transit country. The visa procedure is therefore completed at the German Embassy in Islamabad. The German mission abroad and the other authorities involved will try to complete each procedure quickly.
